What do I do if the appointment form is not opening?
Answer
After selecting the white box on the schedule, the Appointment Form will open in a new or second window. The new/second window might automatically minimize instead of showing up on the screen. If you do not immediately see the new window, please check to see if a second window is available by hovering over the icon for your selected browser.
